DENGUE

Pathological Point Of View -


  • Dengue Fever, Also Known As Breakbone Fever, Is A Mosquito-Borne Tropical Disease Caused By The Dengue Virus.
  • Symptoms Include Fever, Headache, Muscle And Joint Pains, And A Characteristic Skin Rash That Is Similar To Measles.
  • In A Small Proportion Of Cases The Disease Develops Into The Life-Threatening Dengue Haemorrhagic Fever, Resulting In Bleeding, Low Levels Of Blood Platelets And Blood Plasma Leakage, Or Into Dengue Shock Syndrome, Where Dangerously Low Blood Pressure Occurs.

Cause :-

Virology

  • Dengue Fever Virus (DENV) Is An RNA Virus Of  The Family Flaviviridae; Genus Flavivirus.
  • The Dengue Virus Genome (Genetic Material) Contains About 11,000 Nucleotide Bases, Which Code For The Three Different Types Of Protein Molecules (C, prM and E) That Form The Virus Particle And Seven Other Types Of Protein Molecules (NS1, NS2a, NS2b, NS3, NS4a, NS4b, NS5) That Are Only Found In Infected Host Cells And Are Required For Replication Of The Virus.
  • There Are Five Strains Of The Virus, Called Serotypes, Of Which The First Four Are Referred To As DENV-1, DENV-2, DENV-3 And DENV-4.
  • The Distinctions Between The Serotypes Is Based On Their Antigenicity.

Transmission

  • Dengue Virus Is Primarily Transmitted By Aedes Mosquitoes, Particularly A. Aegypti.

  • These Mosquitoes Usually Live Between The Latitudes Of 35° North And 35° South Below An Elevation Of 1,000 Metres (3,300 ft).
  • They Typically Bite During The Day, Particularly In The Early Morning And In The Evening, But They Are Able To Bite And Thus Spread Infection At Any Time Of Day All During The Year.
  • Other Aedes Species That Transmit The Disease Include A. Albopictus, A. Polynesiensis And A. Scutellaris. 
  • Humans Are The Primary Host Of The Virus, But It Also Circulates In Nonhuman Primates. 
  • An Infection Can Be Acquired Via A Single Bite. 
  • A Female Mosquito That Takes A Blood Meal From A Person Infected With Dengue Fever, During The Initial 2–10 Day Febrile Period, Becomes Itself Infected With The Virus In The Cells Lining Its Gut. 
  • About 8–10 Days Later, The Virus Spreads To Other Tissues Including The Mosquito's Salivary Glands And Is Subsequently Released Into Its Saliva. 
  • The Virus Seems To Have No Detrimental Effect On The Mosquito, Which Remains Infected For Life.
  • Aedes Aegypti Prefers To Lay Its Eggs In Artificial Water Containers, To Live In Close Proximity To Humans, And To Feed On People Rather Than Other Vertebrates.
  • Dengue Can Also Be Transmitted Via Infected Blood Products And Through Organ Donation. 
  • In Countries Such As Singapore, Where Dengue Is Endemic, The Risk Is Estimated To Be Between 1.6 And 6 Per 10,000 Transfusions.
  • Vertical Transmission (From Mother To Child) During Pregnancy Or At Birth Has Been Reported. 
  • Other Person-To-Person Modes Of Transmission Have Also Been Reported, But Are Very Unusual.
  • The Genetic Variation In Dengue Viruses Is Region Specific, Suggestive That Establishment Into New Territories Is Relatively Infrequent, Despite Dengue Emerging In New Regions In Recent Decades.

Predisposition
  • Severe Disease Is More Common In Babies And Young Children, And In Contrast To Many Other Infections It Is More Common In Children That Are Relatively Well Nourished.
  • Other Risk Factors For Severe Disease Include Female Sex, High Body Mass index, And Viral Load. 
  • While Each Serotype Can Cause The Full Spectrum Of Disease, Virus Strain Is A Risk Factor.
  • Infection With One Serotype Is Thought To Produce Lifelong Immunity To That Type, But 
    Only Short Term Protection Against The Other Three. 
  • The Risk Of Severe Disease From Secondary Infection Increases If Someone Previously Exposed To Serotype DENV-1 Contracts Serotype DENV-2 Or DENV-3, Or If Someone Previously Exposed To DENV-3 Acquires DENV-2. 
  • Dengue Can Be Life-Threatening In People With Chronic Diseases Such As Diabetes And Asthma.
Mechanism
  • When A Mosquito Carrying Dengue Virus Bites A Person, The Virus Enters The Skin Together With The Mosquito's Saliva. 
  • It Binds To And Enters White Blood Cells, And Reproduces Inside The Cells While They Move Throughout The Body. 
  • The White Blood Cells Respond By Producing A Number Of Signaling Proteins, Such As Cytokines And Interferons, Which Are Responsible For Many Of The Symptoms, Such As The Fever, The Flu-Like Symptoms And The Severe Pains. 
  • In Severe Infection, The Virus Production Inside The Body Is Greatly Increased, And Many More Organs (Such As The Liver And The Bone Marrow) Can Be Affected. 
  • Fluid From The Bloodstream Leaks Through The Wall Of Small Blood Vessels Into Body Cavities Due To Capillary Permeability. 
  • As A Result, Less Blood Circulates In The Blood Vessels, And The Blood Pressure Becomes So Low That It Cannot Supply Sufficient Blood To Vital Organs. 
  • Furthermore, Dysfunction Of The Bone Marrow Due To Infection Of The Stromal Cells Leads To Reduced Numbers Of Platelets, Which Are Necessary For Effective Blood Clotting; This Increases The Risk Of Bleeding, The Other Major Complication Of Dengue Fever.

Pathological Point Of View -

   



  • Fever (Also Known As Pyrexia Or Febrile Response) Is One Of The Most Common Medical Signs And Is Characterized By An Elevation Of Body Temperature Above The Normal Range Of 36.5–37.5 °C (97.7–99.5 °F) Due To An Increase In The Temperature Regulatory Set-Point.
  • This Increase In Set-Point Triggers Increased Muscle Tone And Chills.
  • As A Person's Temperature Increases, There Is, In General, A Feeling Of Cold Despite An Increase In Body Temperature.
  • Once Body Temperature Has Increased To The New Set-Point Temperature, There Is A Feeling Of Warmth.
  • A Wide Range For Normal Temperatures Has Been Found.
  • Temperature In The Anus (Rectum/Rectal) Is At Or Over 37.5–38.3 °C (99.5–100.9 °F)
  • Temperature In The Mouth (Oral) Is At Or Over 37.7 °C (99.9 °F)
  • Temperature Under The Arm (Axillary) Or In The Ear (Otic) Is At Or Over 37.2 °C (99.0 °F)
     


Types :-



Continuous Fever : Temperature Remains Above Normal Throughout The Day And Does Not Fluctuate More Than 1 °C In 24 Hours, e.g. Lobar Pneumonia, Typhoid, Urinary Tract Infection, Brucellosis, Or Typhus. Typhoid Fever May Show A Specific Fever Pattern (Wunderlich Curve Of Typhoid Fever), With A Slow Stepwise Increase And A High Plateau. (Drops Due To Fever-Reducing Drugs Are Excluded.)

Intermittent fever : The Temperature Elevation Is Present Only For A Certain Period, Later Cycling Back To Normal, e.g. Malaria, Kala-Azar, Pyaemia, Or Septicemia. Following Are Its Types :-

    • Quotidian Fever, With A Periodicity Of 24 Hours, Typical Of Plasmodium Falciparum Or Plasmodium Knowlesi Malaria 

    • Tertian Fever (48 Hour Periodicity), Typical Of Plasmodium Vivax Or Plasmodium Ovale Malaria 

    • Quartan Fever (72 Hour Periodicity), Typical Of Plasmodium Malariae Malaria. 
    Remittent fever : Temperature Remains Above Normal Throughout The Day And Fluctuates More Than 1 °C In 24 Hours, e.g., Infective Endocarditis.

    Pel-Ebstein Fever : A Specific Kind Of Fever Associated With Hodgkin's Lymphoma, Being High For One Week And Low For The Next Week And So On. However, There Is Some Debate As To Whether This Pattern Truly Exists.

    A Neutropenic Fever, Also Called Febrile Neutropenia, Is A Fever In The Absence Of Normal Immune System Function. Because Of The Lack Of Infection-Fighting Neutrophils, A Bacterial Infection Can Spread Rapidly; This Fever Is, Therefore, Usually Considered To Require Urgent Medical Attention. This Kind Of Fever Is More Commonly Seen In People Receiving Immune-Suppressing Chemotherapy Than In Apparently Healthy People.

    Febricula Is An Old Term For A Low-Grade Fever, Especially If The Cause Is Unknown, No Other Symptoms Are Present, And The Patient Recovers Fully In Less Than A Week.

    Pathophysiology :-




    • Temperature Is Ultimately Regulated In The Hypothalamus. A Trigger Of The Fever, Called A Pyrogen, Causes A Release Of Prostaglandin E2 (PGE2).
    • PGE2 Then In Turn Acts On The Hypothalamus, Which Generates A Systemic Response Back To The Rest Of The Body, Causing Heat-Creating Effects To Match A New Temperature Level.


    • In Many Respects, The Hypothalamus Works Like A Thermostat. 
    • When The Set Point Is Raised, The Body Increases Its Temperature Through Both Active Generation Of Heat And Retaining Heat. 
    • Vasoconstriction Both Reduces Heat Loss Through The Skin And Causes The Person To Feel Cold.
    • If These Measures Are Insufficient To Make The Blood Temperature In The Brain Match The New Setting In The Hypothalamus, Then Shivering Begins In Order To Use Muscle Movements To Produce More Heat. 
    • When The Fever Stops, And The Hypothalamic Setting Is Set Lower; The Reverse Of These Processes (Vasodilation, End Of Shivering And Nonshivering Heat Production) And Sweating Are Used To Cool The Body To The New, Lower Setting.
    • This Contrasts With Hyperthermia, In Which The Normal Setting Remains, And The Body Overheats Through Undesirable Retention Of Excess Heat Or Over-Production Of Heat. 
    • Hyperthermia Is Usually The Result Of An Excessively Hot Environment (Heat Stroke) Or An Adverse Reaction To Drugs. 
    • Fever Can Be Differentiated From Hyperthermia By The Circumstances Surrounding It And Its Response To Anti-Pyretic Medications.

    General Measures :-


    • Cold Compresses Or Ice Bag - Over Forehead, Axillae, Neck Etc.
    • Bed Rest With Blanket.
    • Semi Solid Or Liquid Diet, Avoid Cold Water.
    • Avoid Exposure To Wind.
    • Avoid Bath - Only Sponging.

    • Most Fever Encountered In General Practice Are Viral, Self Limiting And Require Only Symptomatic Treatment.
    Symptomatic Treatment :-
    • Analgesics.
    • NSAIDs.
    • Anti Histaminics.

    Some Points To Be Kept In Our Mind While Handling The Case Of Fever :- 

    • Before Labelling A Fever As Viral, You Must Look For (As A Compulsory Routine) Pallor, Jaundice Neck Stiffness, Abdomen For Liver & Spleen And Auscultate The Chest.
    • If There Is A Running Epidemic e.g., Malaria, Influenza, Viral Hepatitis Or Typhoid In The Locality, Then That Cause Must Be Thought Of First.
    • Though Many Of The Fevers In General Practice Are Viral And Harmless, The Practitioner Must Be Aware Of The Common Causes Of Fever, And The Symptoms & warning Signals Suggesting That This May Not Be A Simple Fever. You Should Always Try To Find The Cause Of The Fever By Evaluating The Associated Symptoms.
    Fever With Chills :-

    Think Of Malaria And Urinary Infection :-
    • In Endemic Areas, Rule Out Filariasis, (And If No Cause Is Apparent, Think Of Hidden Abscess) Ask For Blood Smear For M.P., Urine, Hb% & WBC.
